When it comes to fitness:
Take time to tend to old injuries as you get your fitness back up to where it was. Do half of the days and amount of time you used to spend working out. Slowly add things back in as we go! If your general fitness has gone down, and you try to pick right back up where you left off, that is a recipe for disaster.
If you have an injury of sorts, use some rehab style exercises to get back into the swing of things, and do right by your body and joints in the process. Then, slowly ramp it up from there. Focus on learning to enjoy the process. Maybe set a new goal, or try a new class.
